1. Click on ‘Add question’ button  .

2. Choose ‘Barcode’ type from the type list (appears on the right of the screen ).

3. Write your chosen title for the question .

 Barcode type question has the following optional settings:

Required 

To be Checked if the question must be answered in the checklist.

Hidden

To be Checked to hide the question in the checklist.

Exclude Report

To be Checked if  you want to exclude the question from the checklist report .
