Create And configure Yes/No Type Question
Click on ‘Add question’ button .
Choose ‘Yes/No’ type from the type list (appears on the right of the screen ).
Write your chosen title for the question .
Question configuration :
Yes/No type question has the following optional settings:
Required
To be Checked if the question must be answered in the checklist.
Hidden
To be Checked to hide the question in the checklist.
Exclude Report
To be Checked if you want to exclude the question from the checklist report .
Require Media
To be Checked if you want the user to attach a media file with an answer .
Require Note
To be Checked if you want the user to append a note with an answer .
Scoring
To be Checked if you want the question answer to be counted in the checklist total score.
Yes/No Options :
You can add your options by click on ‘‘Add Option’ button
